The Federal Death Penalty Resource Counsel requests that counsel with new cases (or a case which has not been brought to the attention of one of the Resource Counsel), complete the form available below (in PDF format). You can either print the form and fill it out by hand, or you can type directly onto the form itself and then print. However, you will not be able to save the completed form unless you have the full version of Adobe Acrobat. Once the form is completed, please send it and the indictment(s) to Kevin McNally by email, fax, or mail (contact information can be found on the second page of the form).
